Your clinicians and their affiliated medical group may use and disclose your health information for treatment, payment and healthcare operations — for example sharing a prescription with a licensed pharmacy, sending a lab order to a CLIA-certified lab, or a referral to an imaging centre.
Your rights
- Get a copy of your health record
- Ask us to correct your record
- Request confidential communications
- Ask us to limit what we share
- Get a list of those with whom we’ve shared information
- File a complaint if you believe your rights were violated
